Abstract

Purpose: To report a rare case of left parasagittal frontal lobe meningioma which presented as acute onset unilateral ptosis without any other neurological deficit in the body.
Case description :40 year female presented with acute onset right eye moderate ptosis with visual acuity 6/24 in both eyes with normal reactive pupils, bilateral grade4 disc edema. Within 2 days the patient developed bilateral restriction of ocular movements suggestive of partial third nerve palsy. MRI revealed a large lesion along the left frontal lobe in parasagittal region with a significant midline shift ,most likely meningioma. However at presentation the patient had only neuro-ophthalmic involvement and no other neurological deficit in body. The tumor was surgically excised & histopathology confirmed the diagnosis. Immediate post-operatively, there was paresis of right hand which gradually improved with marked improvement in ptosis, but the visual acuity remains very poor with optic atrophy setting in.
